1910.184(i)(1) Sling identification. Each sling shall be marked or coded to show the rated capacities for each type of hitch and type of synthetic web material. (a) Finish House: On or about May 25, 2022 and times prior, employees lifted rolls using a hoist with a spreader bar that utilized ratchet straps connected to the load. Employees were in close proximity to the suspended load and were exposed to potential struck-by hazards from failure of the ratchet straps that were not intended to be used for lifting.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Tower Pits: On or about April 23, 2022 and times prior, employees would enter the tower pit to clean scrap without first blocking the carriage above from any potential gravitational energy. (b) Ovens: On or about April 29, 2022 and times prior, the employers would enter the ovens to fix line breaks without first isolating the ovens from its thermal source.
