29 CFR  1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  2377 Boston Rd., Wilbraham: On or about May 31, 2022,  employees were observed using ladders  that were not set at the proper angle and the employer had not ensured that employees using  portable extension ladders to access an upper level were installed in manner in which the working angle did not exceed a ratio of 4:1 or one quarter the working length of the ladder. The ladder was set up at a horizontal distance  greater than 1/4 working length of the ladder,  exposing employees to ladder displacement and fall from height hazards.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR  1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not  protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:   2377 Boston Rd., Wilbraham: On or about May 31, 2022,  employee(s) were observed performing roofing activities on a steep slope commercial building  with unprotected sides and edges greater than 12 feet to the next lower level and were not protected by either guard rails, safety net systems or personal fall arrest systems exposing them to fall from height hazards.  MSL Construction, Inc was previously cited for a substantially similar violation of this occupational safety and health standard, 29 CFR 1926.501(b)13, which was contained in OSHA inspection number   1563098 , citation number 1, item number 1 and was affirmed as a final order on 3/17/22, with respect to a workplace located at  238 Holyoke St., Ludlow, MA.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
