On May 27, 2022, employees of T.S. Banze Construction, Inc. were constructing a four-story apartment complex located at 364 Woodstone Drive building #3, Saint Charles, Missouri. Employees were constructing the floor of the third story, approximately 20 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed working near the edge of the building, exposed to fall hazards.

Banze Construction, Inc. were constructing a four-story apartment complex located at 364 Woodstone Drive, building #3, Saint Charles, Missouri. Employees were constructing the floor of the third story apartment building. Employees were using a 16.5-inch scaffold plank to cross over a 10-foot breezeway located between sections of the building. Employees were exposed to a fall hazards of 10 feet above the lower level.

A Skytrak 6036, Serial # I0358, was used to lift a pallet of lumber to the elevated floor under construction. The employer did not ensure that employees were trained before they operated a powered industrial truck.
