29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or before 6/1/2021, the employer did not ensure that the Samco reel machine (model # SU6000S), where employees were working, utilized adequate machine guarding method such as but not limited too; barrier guards, or electronic safety devices, resulting in employees being exposed to rotating parts such as shafts, and rotating spindle.

29 CFR  1910.217(c)(2)(i):Every point of operation guard shall meet the following design, construction, application, and adjustment requirements:  (a) It shall prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard; (b) It shall conform to the maximum permissible openings of Table O-10; (d) It shall utilize fasteners not readily removable by operator, so as to minimize the possibility of misuse or removal of essential parts;  a) On or about 5/23/2022 the employer did not ensure that the point of operation on the Johnson #317 (model # 27W ) part revolution power press was properly guarded to prevent employees from reaching through, over, under or around guard.  In that the part exit opening measured (7 1/2" X 4 3/4") allowing employees the ability to reach in past the guard.    b) Employees were able to remove the fasteners holding the guards in place with a common 3/16 allen wrench allowing access the point of operation, exposing employees to a caught in hazard.  c) On or about 5/23/2022 the employer did not ensure that the point of operation on the Rousselle #313 (model # 6B) part revolution power press were properly guarded to prevent employees from reaching through, over, under or around guard. in that employees were able to remove the fasteners holding the guards in place with a common 3/16 allen wrench allowing access the point of operation, exposing employees to a caught in hazard.

29 CFR  1910.217(c)(2)(iv):An interlocked press barrier guard shall be attached to the press frame or bolster and shall be interlocked with the press clutch control so that the clutch cannot be activated unless the guard itself, or the hinged or movable sections of the guard are in position to conform to the requirements of Table O-10.  a) On 5/23/2022 the employer did not ensure that the Johnson #317 (model # 27W ) part revolution press had an interlocked press barrier guard attached to the frame or bolster plate to prevent employees from opening the guards by removing the common fasteners  with a 3/16 allen wrench and accessing the point of operation therefore exposing employee to a caught in hazard.

29 CFR  1910.217(e)(1)(i)(A): It shall be the responsibility of the employer to establish and follow a program of periodic and regular inspections of his power presses to ensure that all their parts, auxiliary equipment, and safeguards are in a safe operating condition and adjustment. The employer shall maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected.  (A) Conduct periodic and regular inspections of each power press to ensure that all of its parts, auxiliary equipment, and safeguards, including the clutch/brake mechanism, anti repeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment;  a) On or before 6/1/2022 the employer did not ensure that sufficient periodic inspection were being conducted on power presses such as but not limited to the Johnson #317 and the Roussell #6B, (model #s 27W and 6B60) in that the inspections did not discover that some of the bolts on the guards were missing, that the bolts could be removed by a common 3/16 allen wrench and that the part exit opening allowed employees to excess to the point of operation.
