29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                            a) Exterior, Roof Level of house: Employees were exposed to eye/face injuries while using pneumatic nail guns to install shingles, were not protected with any type of eye/face protection, on or about 5/31/22.   b) Exterior, ladder jack scaffold: Employees were exposed to eye/face injuries while using pneumatic nail guns to install shingles, were not protected with any type of eye/face protection, on or about 5/31/22.    c) Exterior, ladder jack scaffold: Employees were exposed to eye/face injuries while using pneumatic nail guns to install shingles, were not protected with any type of eye/face protection, CSHOs observed this instance at 3:12pm,  on or about 5/31/22.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) Building Exterior Roof: Employees were exposed to a 12 foot fall hazard while installing roof shingles  from a ladder jack scaffold, without the utilizing personal fall arrest system or any other means of fall protection in place, on or about 5/31/22.
