On or about June 6, 2022, at 3376 Bayshore Blvd NE, St. Petersburg, FL, 33702, the green scaffold used by an employee to install steel beam tubes had no guardrail system installed nor was a personal fall arrest system used, exposing the employee to a 10 ft. 2 inch fall hazard.

On or about June 6, 2022, at the job site,  the yellow scaffold systems was not inspected prior to use, in that, employees were exposed to fall hazards due to deficiencies in the construction and installation of supported scaffold while performing beam installation.  b.) On or about June 6, 2022, at the job site,  the green scaffold systems was not inspected prior to use, in that, employees were exposed to fall hazards due to deficiencies in the construction and installation of supported scaffold while performing beam installation.

29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a.) On or about June 6, 2022, at the worksite, two employees conducted beam reinstallation while on the yellow scaffold that was not fully planked between the front uprights, therefore exposing employees to a 10 ft. 2in fall hazard.  b.)
