29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained and kept free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, ice and snow. Building 1523 Lobby: On and/or before June 3rd 2022 the beverage vending machine to the left-hand side of the entrance was leaking water onto the direct path that employees used to walk to get in and out of building 1523. Employees in the immediate vicinity using the machine for refreshments and employees walking to and from offices were exposed to slips and falls.

29 CFR 1910.303(g)(1)(ii):The required working space about electric equipment rated 600 volts, nominal, or less was used for storage: On and/or before June 3rd, 2022 the employer permitted materials to be stored directly in front of the electrical panel(s). This condition interferes with access and immediate shutdown of a circuit in the panel, particularly in the event of an emergency. Employee(s) were exposed to the potential of shock and/or fire hazard(s) In the following locations: A) Las Pulgas Gate Duty Hut: Ready access to the circuit breaker panel was blocked by trash bag, police gear and radio charging storage box. B) San Luis Rey Gate: Ready access to the circuit breaker panel was blocked by police gear such as bags and PPE equipment.

29 CFR 1910.334(a)(2)(ii): When there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electric equipment safe had been made: San Onofre Gate duty hut: On and/or before June 3rd 2022, a relocatable power tap (power strip) had one outlet not secured in place by holding screw that became loose and receded into its housing. Communication radio is directly connected to the power strip which exposes police officers to electrical hazards.
