29 CFR 1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) EMJD Corporation: On or before 6/10/2022 the employer did not ensure that aftermarket attachments used on the Hyster 65 forklift were approved by the manufacturer and changes reflected in a new data plate. The employer used 66" fork extenders which were not approved by the manufacturer. The changes in load center were not reflected in the forklift's data plate. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) EMJD Corporation: On or before 6/10/2022 the employer did not protect employees from injury in that the Pedrazzoli horizontal bandsaw lacked guarding on the unused portion of the blade outside the clamping area. Approximately 6 inches of blade was exposed. This condition exposed employees to amputation and laceration hazards. Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
