29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  (a) On or about November 17, 2022, employees were not protected from a tripping hazard while walking in and around the maintenance shop in that a section of grating located in the East corner outside of bay 8 had 3 cross section pieces of metal broken free from their welds.  (b) On or about November 17, 2022, employees were not protected from a tripping hazard while walking in and around the maintenance shop in that a section of grating located outside on the North side of the maintenance shop had pieces of metal broken free from their welds and missing.  (c) On or about November 17, 2022, employees were not protected from a tripping hazard while walking in and around the maintenance shop in that a section of grating located outside of trailer maintenance bay 1 on the East side of the maintenance shop was bowed up into the air approximately 1.5 inches on one end.  (d) On or about November 17, 2022, employees were not protected from a tripping hazard while walking in and around the maintenance shop in that a section of grating located outside of tractor maintenance bay 2 on the North side of the maintenance shop was bowed up into the air approximately 2 inches on one end.

29 CFR 1910.22(d)(2): Hazardous conditions on walking-working surfaces were not corrected or repaired before an employee uses the walking-working surface again. If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired:  (a) On or about June 9, 2022, employees were not protected from a loss-of-control hazard while required to operate powered industrial trucks on the concrete dock and ramp that had potholes and protruding metal objects on it that were not repaired or guarded.  (b) On or about November 17, 2022, employees were not protected from a tripping-hazard while required to walk and work on the concrete floor within the shop and the parking areas on the outside that had potholes and unevenness that was not repaired or guarded.

29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee was protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  On or about June 10, 2022, employee(s) were not protected from floor holes in that: (a) The grating covering a drain on the outside corner of the shop at the South end of bay 8 had an approximately 4-inch-wide gap exposing employees to a step-into hazard. (b) The grating covering a drain inside of the shop on the North end between bay 7 & 8 had collapsed inward approximately 1.5 inches and was no longer flush with the floor exposing employee(s) to a tripping hazard.

29 CFR 1910.215(a)(4): Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage:  (a) On or about June 9, 2022, a work rest on a grinder located in between bays 9 and 10 was not adjusted within one-eighth of an inch from the grinding wheel exposing employees to a caught-in hazard.  (b) On or about November 17, 2022, a work rest on a grinder located in the Northeast corner of trailer maintenance bay 4 was not adjusted within one-eighth of an inch from the grinding wheel exposing employees to a caught-in hazard.  Central Transport was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(a)(4), which was contained in OSHA inspection number 1514613, citation number 1, item number 1 and was affirmed as a final order on March 18, 2021, with respect to a workplace located at 615 Miami Ave., Kansas City, KS. 66105.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  On or about June 9, 2022, employees were not protected from electrical shock hazards in that: (a)	On the West wall between bays 9 & 10 there was a 4-receptacle electrical box with live wiring exposed due to conduit being pulled from its fitting. (b)	On the East wall between bays 9 & 10 there was an electrical box with no cover on it and live bare electrical wires were exposed. (c)	On the East wall by bay 9 garage door a two-receptacle electrical box had been pulled away from its base mount exposing live electrical wiring. (d)	On the East wall within bay 4 an electrical junction box was missing its cover exposing live electrical wiring.
