(d) 158 West Jackson St, New Holland, PA, Packaging, 331 Bright Stacker - On or about July 14, 2022, employees were exposed to caught in hazards while working near the suction arm intake section which was approximately 42.5 inches above the ground level and the opening was approximately 45.5 inches from the table where the cardboard is placed to the bottom of the suction arm;  and also at the approximately 41 inch wide platform with 3 1-inch wide exit conveyor chain pallet movers where employees work within 1 foot of the rotating chain.   (e) 158 West Jackson St, New Holland, PA, Packaging, 332 Bright Stacker - On or about July 14, 2022, employees were exposed to caught in hazards while working near ingoing nip points caused by the approximately 41 inch wide platform which was approximately 14 inches above the floor level with 3 1-inch wide exit conveyor chain pallet movers.   (f) 158 West Jackson St, New Holland, PA, Packaging, IMPACO Robot - On or about July 14, 2022, employees were exposed to caught in hazards while working near ingoing nip points caused by the 3 1-inch wide exit conveyor chain pallet movers which were approximately 24 inches above the floor level with. (b) 158 West Jackson Street, New Holland PA, Lower Engine Room - On or about July 13, 2022, the knife switch disconnect for Condenser #1 next to the water surge tank was not readily accessible due to pipes and stored materials obstructed access and being located through a doorway that is not easily accessible. Employees were exposed to caught in hazards if attempting to access the disconnect switch and electrical hazards if the employee needs the electricity disconnected in the case of an emergency while working on Condenser #1.   (c) 158 West Jackson Street, New Holland PA, Mix Area - On or about July 14, 2022, the 9 knife switch disconnects for the blender and pumps in the mix area were not readily accessible as they were elevated to a height that would require the use of a ladder or other means to access, exposing employees to electrical and caught in hazards.   (d) 158 West Jackson Street, New Holland PA,  Washdown Room EMS - On or about July 19, 2022, the 8 knife switch disconnects for the Blender,  PP 404, AG 300B, AG 300A, AG 200B, AG 200A, AG 100A, CP 506, were not readily accessible as they were elevated to a height that would require the use of a ladder or other means to access them, exposing employees to electrical and caught in hazards.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
