29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit:    a. Hassia Auto-Stacker - On or about January 26, 2024, Prior to the start of work to change the motor from Goldco Industries Model H3060 auto stacker lift #1 to #2 an entry permit was not filled out prior to the employees entering into the permit required confined space, exposing to caught in and struck by hazards.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including but not limited to, the following: Specifying acceptable entry conditions; Isolating the permit space;    a. Hassia Auto-Stacker - On or about January 26, 2024, the employees engaged in changing the motor from Goldco Industries Model H3060 auto stacker lift #1 to #2 entered into the permit required confined spaced without having procedures or practices in place to safely enter the space and isolate the permit space to minimize the hazardous conditions entrants would face.  Advanced Food Products LLC was previously cited for a violation of this occupational safety and health standard 1910.146(d)(3), which was contained in OSHA inspection number 1601440, citation number 1, item number 4a and was affirmed as a final order on 12/21/2022, with respect to a workplace located at 158 West Jackson Street, New Holland, PA 17557.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146:    a. Hassia Auto-Stacker - On or about January 26, 2024, maintenance employees assigned to perform work involving permit required confined space entry were not provided training on the employer's program, procedures, rescue and authorized entrant and attendant duties prior to entering into a permit required confined space.  The employees were exposed to caught in and struck by hazards.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Hassia Auto-Stacker - On or about January 26, 2024, the employees engaged in changing the motor from Goldco Industries Model H3060 auto stacker lift #1 to #2 were not provided or assigned lock out tagout devices to allow each authorized employee to lock out the electrical, hydraulic, mechanical and gravity energy sources of the equipment prior to entering the danger area, the employees were exposed to caught in and struck by hazards. 29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  a. Hassia Auto-Stacker - On or about January 26, 2024, the employees engaged in changing the motor from Goldco Industries Model H3060 auto stacker lift #1 to #2 did not lock out the electrical, hydraulic, mechanical and gravity energy sources of the equipment prior to entering the danger area, the employees were exposed to caught in and struck by hazards.   Advanced Food Products LLC was previously cited for a violation of this occupational safety and health standard 1910.147(d)(3), which was contained in OSHA inspection number 1509028, citation number 1, item number 1b and was affirmed as a final order on 07/20/2021, with respect to a workplace located at 600 1st Avenue West, Clear Lake, WI 54005.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
