29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. Pacifico Builder LLC. at 6087-6089 N. Liverpool St Aurora, CO 80019: On June 13, 2022, the exposing employer did not ensure that employees were protected from injury, in that portable ladders, used to access the first level a residential home, had side rails that did not extend at least 3 feet above the upper landing or was secured. The employer did not protect employees against falls at heights over 10 feet. This condition exposes to employees to head injuries and broken bones.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. Pacifico Builder LLC. at 6087-6089 N. Liverpool St Aurora, CO 80019: On June 13, 2022, the exposing employer did not ensure that employees were protected from injury, in that four employees were observed conducting framing operations at heights over 15 feet on a new residential construction without the use of fall protection. This condition exposes to employees to head injuries and broken bones. Pacifico Builder LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1326891, citation number 1, item number 1 and was affirmed as a final order on August 16, 2018, with respect to a workplace located at 12943 E Briarwood, Centennial, CO 80122.
