29 CFR  1910.23(d)(2): The employer did not ensure that the minimum perpendicular distance from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder is 7 inches (18 cm):  In the following locations, sufficient clearance was not provided behind fixed ladders:  A.	Community Building, Near Cooling Tower:  On, or about, June 16, 2022, the clearance from the centerline of the rungs on the fixed ladder was measured to be six inches from the building surface.  B.	Fleet Building:  On, or about, July 13, 2022, the clearance from the 22nd rung (ascending) was measured to be four inches from the roof edge.

29 CFR  1910.29(b)(2)(i):  The employer did not ensure that guardrails met the requirement that midrails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface:  A.	Main Tower, Roof Access Stairs:  On, or about, June 16, 2022, the upper stair landing provided for the fixed stairs was not provided with a midrail or equivalent member.  B.	Main Tower, Roof Access Landing:  On, or about, June 16, 2022, the midrail provided in the guard rail system installed at the roof landing for the access stairs was installed higher than "midway" between the top rail and roof level, which left a larger opening between the midrail and roof surface.    Abatement Note:  The roof access stairs and landing should be reconfigured to not require employees to navigate down a set of steep stairs onto a membrane roof in close proximity to the roof edge.  As an interim measure, in addition to lowering the midrail, a toeboard or other solid member should be added to the existing guardrail to fill the gap(s) between railings to offer a more positive stop in the event that an employee slips at the lower stair landing.

29 CFR  1910.335(a)(1)(i):  Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:  Facility Maintenance:  On, or about, June 16, 2022, personal protective equipment such as, but not limited to, face shields, a fire resistive balaclava and head protection were not required to be worn by employees who performed live electrical work, including trouble-shooting, or who verified that electrical equipment was properly de-energized.  Section 6.6 of the Safety and Health program did not require the use of an arc rated face shield and balaclava for maintenance and preventative maintenance work that was performed by mechanics on equipment up to and including 480 volt circuits and equipment.
