This condition could lead to serious health conditions.

Additionally, no toilet paper was provided for employee use.  This condition exposed employees to multiple health hazards including virus and bacterial infection.

29 CFR  1910.178(q)(7): Industrial trucks were not examined before being placed in service:  (a)Sioux Laundry, Inc. at 499 Prosperity Way, North Sioux City, SD 57049:  On or about and at times prior to June 14, 2022, the employer failed to inspect a Mitsubishi powered industrial truck (PIT), model # 2218, Serial #AF810-00204 for defects prior to use.  Multiple damaged components were identified and hydraulic oil was observed under the mast of the machine while it was in regular service.  This condition exposed employees to crushed-by and amputation hazards associated with a potential critical forklift component failure.

This condition exposed employees to shock and/or electrocution hazards.
