This condition exposed employees to fire hazards in that it obstructed employee emergency egress from the facility.

29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)Sioux Laundry, Inc. at 499 Prosperity Way, North Sioux City, SD 57049: On or about and at times prior to November 2, 2023, the employer did not ensure employees were protected from caught-in and amputation hazards in that the company had not developed, documented and utilized an energy control  procedure for the Milnor Pellerin Laundry Press Model: MP1603CL. This condition exposed employees to caught-in and amputation hazards.   Abatement Note: Energy Control Procedures shall clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, and the means used to enforce compliance including, but not limited to; -A specific statement of the intended use of the procedure. -Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  -Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. -Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR  1910.147(f)(1)(ii):When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2):  (a)Sioux Laundry, Inc. at 499 Prosperity Way, North Sioux City, SD 57049: On or about and at times prior to November 2, 2023, the employer did not ensure employees were protected from caught-in and amputation hazards in that employees were not removed from the area of the Milnor Pellerin Laundry Press Model: MP1603CL when the equipment was operated in the maintenance only position which bypasses door interlocks and permits access to the press during operations in an attempt to identify a leak on the hydraulic system of the laundry press.  Non-maintenance employees continued to perform work in the vicinity of the laundry press with the equipment operating the the maintenance only position.  This condition exposed employees to caught-in and amputation hazards.
