29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  a)	Sanchez Roofing, on or about June 15,2022 at the address of 2930 Estuary Dr, Maumee Ohio: The employer did not ensure employees were protected from falls while removing old and installing new roofing material atop a residential type structure. Employees operating in this condition were exposed to a 10ft fall to the level below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a)	Sanchez Roofing, on or about June 15, 2022 at the address of 2930 Estuary Dr, Maumee Ohio: the employer Sanchez Roofing did not ensure the side rails of one portable aluminum extension ladder was extended at least three feet above the landing surface. Two employees were exposed to approximately 10 feet fall to the ground below while using the ladder to transition to the roof level.
