29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.    On or about June 16, 2022, during a roof replacement of a residential home located at 13839 Clifton Blvd, Lakewood, OH 44107, the employer did not ensure employees working on the roof were protected from a fall of approximately 19.4 feet by utilizing a personal fall arrest system or any other form of fall protection system.  Cristians Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1500595, citation number 01, item number 002 and was affirmed as a final order on 02/05/2021, with respect to a workplace located next to Homesite 84 Cedar Crest Dr, Elyria, OH 44035.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 16, 2022, during a roof replacement of a residential house located at 13839 Clifton Blvd, Lakewood, OH 44107, employees were using an extension ladder to access a roof. The employer did not ensure the extension ladder extended 3 feet above the roof, exposing employees to fall hazards of approximately 19.4 feet to the ground below while accessing the roof.   Cristians Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1) ladder side rails shall extend at least 3 feet above the upper landing surface), which was contained in OSHA inspection number 1500595, citation number 01, item number 003 and was affirmed as a final order on 02/05/2021, with respect to a workplace located next to Homesite 84 Cedar Crest Dr, Elyria, OH 44035.
