29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about July 8, 2022, during the installation of roofing material at a job site located at 7995 Darrow Rd, Twinsburg, Ohio 44087; the employer did not ensure employees working on a roof of a commercial location were protected from a fall of approximately 30 feet by utilizing a fall protection system.   CRISTIAN'S ROOFING LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(B)(13) Duty to have fall protection), which was contained in OSHA inspection number 1584529, citation number 01, item number 02 and was affirmed as a final order on (6/8/2022), with respect to a workplace located at Lot 10C, 34580 Jude CT, Willoughby, Ohio 44094.  CRISTIAN'S ROOFING LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(B)(13) Duty to have fall protection), which was contained in OSHA inspection number 1602556, citation number 01, item number 01 and was affirmed as a final order on (7/28/2022), with respect to a workplace located at 13839 Clifton Blvd, Lakewood, Ohio 44107.
