29 CFR  1926.403(b)(2): Listed, labeled, or certified equipment was not  installed and used in accordance with instructions included in the listing, labeling, or certification:  a)  2000 Ellington Metro West., Westborough, MA: On or about June 15, 2022,  employees were noted to be using multiple extension cord sets that were connected together to increase the reach of the cords ( aka daisy chaining) which is not permitted by the cord manufacturer and the controlling employer did not ensure that his sub contractor was using extension cords  in accordance with the instructions included in the listing, labeling, or certification.   b) 4000 Ellington Metro West., Westborough, MA: On or about June 15, 2022, employees were noted to be using a ground fault circuit interrupter (GFCI) that was connected (plugged) into extension cord sets instead of an outlet receptacle and the controlling employer did not ensure that his sub contractor was using the GFCI in accordance with the listing, labeling or certification.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR  1926.1053(b)(5)(i):Non-self-supporting ladders were used at an angle such that the horizontal distance from the top support to the foot of the ladder was  greater than approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  the controlling employer did not ensure that his sub contractor's employee(s) using aluminum extension ladders  to access upper levels while  installing siding,  installed the ladders so that the working angle did not exceed a ratio of 4:1 or one quarter the working length of the ladder. The ladder was set up at an angle of nearly 2: 1 exposing employees to ladder displacement and fall from height hazards.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
