(b)  J & M Pallets, LLC, 201 N. Bell St., Nashville, Georgia; in the production area, on or about August 3, 2022 and at times prior to, the employer exposed employees to noise hazards, in that a hearing conservation program was not implemented for an employee exposed to noise at 107% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 85.4 dBA during the 414 minute sampling period.  (c)  J & M Pallets, LLC, 201 N. Bell St., Nashville, Georgia; in the production area, on or about August 3, 2022 and at times prior to, the employer exposed employees to noise hazards, in that a hearing conservation program was not implemented for an employee exposed to noise at 148% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 87.8 dBA during the 409 minute sampling period.  (d)  J & M Pallets, LLC, 201 N. Bell St., Nashville, Georgia; in the production area, on or about August 3, 2022 and at times prior to, the employer exposed employees to noise hazards, in that a hearing conservation program was not implemented for an employee exposed to noise at 171% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 88.8  dBA during the 409 minute sampling period.  J & M Pallets, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1910.95(c)(1)], which was contained in OSHA inspection number 1411797, citation number 1, item number 1 and was affirmed as a final order on December 27, 2019, with respect to another workplace located at 6370 Highway 441 South, Pearson, Georgia.
