Location: Worksite On or about May 27, 2022, an employee fell approximately forty feet to the ground below while working where the employer had not ensured employee protection from fall hazards.

29 CFR 1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) Location: Worksite On or about May 27, 2022, employees were exposed to fall hazards while working on a fabricated steel tube scaffold where the working level was not fully planked.
