1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): (a) On or about 6/17/2022 and at times prior to in the maintenance department, employees were exposed amputation hazards when operating the Cincinnati Mechanical Shear Machine, Model 1808, to cut sheet metal because a safety instruction sign describing the correct placement of the metal stock was not posted.
