29 CFR 1910.179(n)(2)(ii): The load shall be attached to the load block hook by means of slings or other approved devices. (a) On or about June 22, 2022, employee(s) were exposed to a crushing hazard while fabricating a steel hopper, and in the process of welding braces onto the hopper, the hopper was disconnected from the jib crane. The employer had not developed and implemented written procedures to ensure the load was secured by the jib crane or proper bracing was utilized during the entire fabrication process.

1910.254(d)(9)(iii) Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions. (a) On or about June 22, 2022, employees were exposed to electrical hazards while welding with a Miller Deltaweld Welder 451, in-that the welders output cables had bare wires, exposing the employee welding to an electrical shock hazard.
