29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  LOADING DOCK #4: The employer did not provide fall protection on the loading dock where the walking/working surfaces had unprotected sides or edges that were 4 feet or more above a lower level.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  MINI COMM: Employees who were exposed to corrosive chemicals during routine work, including Clorox Bleach, did not have access to facilities for flushing of the eyes, exposing them to injury from contact.

29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and an evaluation of the operator's performance in the workplace.  WAREHOUSE: The employer did not ensure that employees were evaluated before operating electric pallet jacks, exposing employees to struck-by hazards.

29 CFR  1910.37(b)(4):Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  MEAT DEPARTMENT STOREROOM: Exit access signs were not posted above the storeroom sliding door to indicate the direction of travel, exposing employees to fire hazards.
