1910.132(d)(1) The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:  29 CFR  1910.132(d)(1)(i):Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;  a. On or about and prior to June 27, 2022, the employer failed to select personal protective equipment for the upper extremities and torso for protection against splashing chemicals during Coater Room clean up activities. Wide Line coating operators were required to wear flame resistant uniform tops without adequate protection from chemical contact with the organic solvents used in this task. When performing clean up required duties, employees have potential exposure to skin contact hazards related to methyl ethyl ketone and Aromatic 150.

29 CFR  1910.132(f)(1):  The employer shall provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following:  29 CFR  1910.132(f)(1)(iv):  The limitations of the PPE;   a. On or about and prior to June 27, 2022, for Coater Room employees who performed clean up activities, the employer failed to provide training on the limitations of flame resistant uniforms as protection against organic solvents and the limitations of the flame resistant clothing when combustible material penetrates into the fabric of the uniform as protection against flash fire hazards. Organic solvents, including Aromatic 150 and methyl ethyl ketone are used in cleaning Coater Room equipment, including but not limited to cleaning rollers.

29 CFR  1910.133(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about and prior to June 27, 2022,  the employer failed to ensure that employees used face protection during Coater Room clean up activities. Employees have exposure to splash from liquid organic solvents during   Coater Room cleaning activities, such as but not limited to, cleaning of  the rollers.

29 CFR  1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  a. On or about June 27, 2022, the employer failed to verify through a written certification that the workplace hazard assessment for the coating line Coater Room clean up activities had been performed. During Coater Room clean up activities, employees use organic solvents for cleaning surfaces, such as mechanical roller components, on a regular basis resulting in skin contact hazards to the hands, arms and face.
