29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  a) On or about March 9, 2026, located in the Production Department, employees changed rubber rolls on the Narrow Coat Line without having procedures for employees to utilize that provided the necessary specific information as required for the control of hazardous energy. The procedure did not include specific steps for isolating and securing the pneumatic energy [B], placement of lockout devices onto the pneumatic energy [C], and verifying that the pneumatic energy was isolated [D].

29 CFR  1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a) On or about March 9, 2026, located in the Production Department, the employer failed to conduct an adequate periodic inspection of the energy control procedure for the Narrow Coat Line-Wet Section in that energy sources were not identified or controlled, thereby exposing employees to caught-in hazards during squeegee roll changes.

29 CFR  1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a) On or about March 9, 2026, located in the Production Department, employees were exposed to machine hazards associated with Narrow Coat Line-Wet Section while performing servicing and maintenance activities, including changing squeegee rolls. The employer failed to implement energy control application steps as the pneumatic energy was not isolated and controlled prior to performing the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving air isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
