29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about 6/28/2022 employees at the Lichtenstein Financial roofing project at 37 New London Turnpike Glastonbury Connecticut were exposed to falls in the following area: 1. Employees was exposed to approximately 20' exterior falls from the main roof during roofing activities. 2. Employees were exposed to approximately 10' exterior falls from a lower roof while removing roofing debris.

29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about June 28,2022 employees at the Lichtenstein Financial Building roofing project were actively using a 32' Werner aluminum extension ladder to access the roof. The ladder was not extended 3 feet past the landing.
