29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New residential construction located on Lot #352 at 165 Dockside Drive, Fayetteville, GA 30214, Roof: On or about 06/28/2022, the employer exposed employees to fall hazards of approximately 12-feet, 5-inches to the lower ground level, in that a fall protection system was not provided to all employees engaged in roofing activities on a steep 8/12 and 10/12 pitched roof.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) New residential construction located on Lot #352 at 165 Dockside Drive, Fayetteville, GA 30214, Front side of structure: On or about 06/28/2022, the employer exposed employees to fall hazards of approximately 12-feet, 5-inches to the lower ground level, in that a ladder with extended side rails at least 3-feet past the roof eave was not provided to employees accessing the roof and ground levels.
