29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.   228 Hartford Turnpike, Shrewsbury, MA: On March 17, 2022 and at times prior, employees performing  roof sheathing activities  used   reusable roof anchors that were installed incorrectly without the required number and specific type of attachment fasteners  required for proper installation.

29 CFR  1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service.   228 Hartford Turnpike, Shrewsbury, MA: On March 17, 2022 and at times prior, an employee used a  Guardian Aardvark Self-Retracting Lifeline device that was not inspected prior to use for wear, damage and other deterioration. The device in question had defects that would require that it be removed from service and the employer did not remove it from service.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  228 Hartford Turnpike, Shrewsbury, MA: On or about March 17, 2022: employee(s) were exposed to fall hazards as part of their assigned duties and the employer had not trained them prior to the exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards and procedures each employee must take to prevent or minimize risk of such exposures.

The employee's  date of hospitalization was March 17, 2022  and the employee injury was later discovered by OSHA  on June 24, 2022.
