An employee was required to clear a jam on the Seven Chefs Meat Slicer, Model ACM-201U and sustained a middle fingertip amputation on left hand.

On or about June 22, 2022, Packaging Department, the employer failed to ensure  that an authorized employee was assigned the task of clearing a jam on the Seven Chef's Food Slicer Machine, Model No. ECM-201U, and performing an effective lockout of the machine.  A general laborer employee sustained an amputation of left middle fingertip on left hand while clearing a jam, thereby exposing the employee to the unexpected energizing, start up or release of stored energy.
