29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  a) Jobsite located at 250 North Bradford Avenue, West Chester, PA.: The employer did not initiate a program to conduct frequent and regular inspections of the jobsite, to identify hazards and/or stop the work in the event workers were placed in a hazardous condition. Employees performing siding and other related work at the jobsite were exposed to fall hazards.   Condition was observed on or about June 30, 2022.

29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Jobsite located at 250 North Bradford Avenue, West Chester, PA.: The employer did not provide training to an employee working on a pump jack. The employee was engaged in siding activities while working on a pump jack. The employee was not trained on how to recognize, evaluate, and mitigate hazards associated with pump jacks. Including, but not limited to falls.  Condition was observed on or about June 30, 2022

Such activities were not performed only by experienced and trained employees selected for such work by the competent person:  a) Jobsite located at 250 North Bradford Avenue, West Chester, PA.: The employer did not ensure that the pump jack was erected by a competent person who was trained and experienced to supervise the proper assembly of the pump jack. An employee was engaged in siding activities while working from a pump jack; thus exposing the employee to struck-by and fall hazards.  Condition was observed on or about June 30, 2022.
