29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about July 22, 2022, the employer did not develop, document, and utilize/implement procedures for machinery that had more than one hazardous energy source; such as, but not limited to the 533 Tig Sled Welder, Murata 612 and Nakamura 674 CNC machines which have electrical and pneumatic energy sources. This exposed employees to crushing and amputation hazards.
