29 CFR 1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) 9265 Cloud Shadow Lane, Littleton, CO 80125: On or about June 18, 2022, and at times prior, the employer, J and A, LLC., exposed its employees to fall hazards by not ensuring that the personal fall arrest system that was in use was sufficient and rigged to ensure the employee would not free fall more than 6 feet from the roof of a residential building to the ground. The employee fell 24.2 feet from the gable to the ground and was hospitalized for a broken leg and ankle injury requiring surgery.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: 9265 Cloud Shadow Lane, Littleton, CO 80125: On or about June 18, 2022, and at times prior, the employer, J and A, LLC., exposed its employees to fall hazards by not ensuring each employee was sufficiently trained while wearing personal fall arrest system(s). The employees personal fall arrest system was not rigged to ensure the employee would not free fall more than 6 feet from the roof of the residential building to the ground. The employee fell 24.2 feet from the gable of the residential building to the ground and was hospitalized for a broken leg and ankle injury requiring surgery.
