29 CFR  1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  On or about July 06, 2022, employees were exposed to struck by hazards while utilizing an electric saw to complete roof work on a residential home located at 121 Pine Ave Hudson, Ohio 44236.  The employer did not ensure employees were wearing any eye protection.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about July 06, 2022, employees were exposed to fall of approximately 20 feet on a 8/12 pitch atop a residential roof located at 121 Pine Ave Hudson, Ohio 44236.  The employees were working on all areas of the roof and the employer did not ensure they were using any form of fall protection.
