29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about January 18, 2023, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at site 181 Pine Valley Drive, Columbia Station, Ohio 44028.  Employees were working above other employees with tools and materials that could have fallen, and the employer did not ensure the ground employees were wearing protective helmets.

29 CFR  1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about January 18, 2023, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at site 181 Pine Valley Drive, Columbia Station, Ohio 44028.  The employees were using an air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.    Slaubaugh Bros. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.102(a)(1), which was contained in OSHA inspection number 1605969 citation number 1, item number 1, and was affirmed as a final order on 8/24/22, with respect to a workplace located at 121 Pine Ave., Hudson, Ohio 44236.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about January 18, 2023, employees were exposed to a fall of approximately 21 feet while performing roofing activities on a residential home located at site 181 Pine Valley Drive, Columbia Station, Ohio 44028.  Employees were working on all areas of the 2nd floor and the employer did not ensure they were using fall protection.  Slaubaugh Bros. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1605969 citation number 1, item number 2, and was affirmed as a final order on 8/24/22, with respect to a workplace located at 121 Pine Ave., Hudson, Ohio 44236.   Slaubaugh Bros. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1539658 citation number 1, item number 1a, and was affirmed as a final order on 7/1/21, with respect to a workplace located at East 14th and Superior, Cleveland, Ohio 44114
