29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) 5154 Vivian Street Wheat Ridge, CO 80033: On or about June 16, 2022, Axis Construction Services LLC, as a controlling and correcting employer, did not ensure that employees of 4 Hernandez, subcontractor, were safe from health and safety hazards by ensuring employees were using fall protection when working at heights greater than 6 feet. Axis Construction Services LLC was present on site and did not ensure fall protection was utilized during framing operations.
