Eastman AVE., Morrison CO. 80465: On or about July 13, 2023, and at times prior, Axis Construction Services LLC as a controlling employer, did not ensure that effective frequent and regular inspections were conducted on this jobsite so as to ensure that subcontractor employees were protected from hazardous conditions including fall hazards.

Eastman AVE., Morrison CO. 80465: On or about July 13, 2023, and at times prior, Axis Construction Services, LLC as a controlling employer, did not ensure that subcontractor employees were protected from fall hazards when walking around the house. Covers were missing on basement egress windows exposing a fall hazard greater than 6ft in depth.
