Warehouse, Aisle A-B - On or about July 8, 2022, an employee was observed operating Raymond Reach Forklift, Model 730-DR30TT, Serial No. 730-22-BC09642. The employer failed to ensure that employee(s) who operate powered industrial trucks were properly trained, evaluated, and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.

Warehouse A, Aisle A-B - On or about July 8, 2022, an employee observed operating Raymond Reach Forklift, Model 730-DR30TT, Serial No. 730-22-BC09642, was traveling too fast for conditions as three pedestrians approached aisle A-B. The employer failed to ensure that forklift operators observe all traffic regulations, including authorized plant speed.
