Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. Located at an Aqua Man Water Hauling job site at Millersburg Water Sewage Treatment Facility at 1372 S. Washington St., Millersburg, Ohio: An employee was hospitalized on 6/27/2022 as a result of injuries sustained during a workplace incident that occurred on 6/27/2022. The employer failed to report the workplace hospitalization to OSHA within 24 hours.
