29 CFR 1926.1053(b)(6):Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement. On or about July 7, 2022, an employee was exposed to a fall of approximately 8 feet while removing a dead cable line located at 15214 Corkhill, Mayfield Heights, Ohio 44137. The employee was using an extension ladder to access the cable and the employer did not ensure that it was set up to prevent displacement.
