29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) 1100 Grandon Way, Mechanicsburg, Pennsylvania - An employee suffered a work-related injury on June 29, 2022 as a result of tripping on a mat and falling to the floor. The injury required hospitalization, and the employer failed to report this work-related hospitalization to OSHA within 24 hours. No abatement certification or documentation required
