29 CFR  1926.25(a):During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  At a worksite located at Creekside Village Townhomes in Parkville MO -- debris, trash and materials caused trip hazards all around and throughout the worksite. This exposed employees to slip, trip and falls. This was documented on or about 07/12/2022.

29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:Note:  Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift.  At a worksite located at Creekside Village Townhomes, Parkville MO -- employee engaged in framing duties was standing on a load of plywood that was raised via an off-terrain forklift. The employee was exposed to fall hazards in that the platform was not designed and/or to be used as a scaffold platform. This was documented on or about 07/12/2022.

The employee was exposed to fall hazards in that the platform was not designed and/or to be used as a scaffold platform. This was documented on or about 07/12/2022.

This exposed the employees to debris getting in their eyes. This was documented on or about 07/12/2022.  Jeovanni Wilson Miranda dba Miranda Framing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1411049, citation number 2, item number 1 and was affirmed as a final order on 06/26/2019, with respect to a workplace located at 3909 W 85th St, Prairie Village KS 66206.  Jeovanni Wilson Miranda dba Miranda Framing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1190332, citation number 1, item number 3 and was affirmed as a final order on July 07, 2017, with respect to a workplace located at 5346 Hedge Lane Court Shawnee, Kansas.
