29 CFR  1926.25(a):During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  At a worksite located at Creekside Village Townhomes in Parkville MO -- debris, trash and materials caused trip hazards all around and throughout the worksite. This exposed employees to slip, trip and falls. This was documented on or about 07/12/2022.

29 CFR  1926.404(b)(1)(iii)(C):Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  At a residential worksite located at Creekside Village Townhomes, Parkville, MO -- employees engaged in framing duties were using an electrical cord with a missing ground pin to power an air compressor. This exposed employees to possible electrical shocks and / or burns. This was documented on or about 08/22/2022.

29 CFR  1926.102(a)(1):Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At worksite located at Creekside Village Townhomes in Parkville MO -- employees engaged in framing duties using hammers and pneumatic nail guns were not wearing eye protection. This exposed the employees to debris getting in their eyes. This was documented on or about 07/12/2022.  Jeovanni Wilson Miranda dba Miranda Framing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1411049, citation number 2, item number 1 and was affirmed as a final order on 06/26/2019, with respect to a workplace located at 3909 W 85th St, Prairie Village KS 66206.

This exposed employees to fall hazards greater than 2 stories to ground level. This was documented on or about 07/12/2022.    Jeovanni Wilson Miranda dba Miranda Framing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1411049, citation number 1, item number 3, and was affirmed as a final order on 1/16/2020, with respect to a workplace located at 3909 W 85th St, Prairie Village KS 66206.
