29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) 5278 Brush Valley Road Rebersburg, Pennsylvania, - Employees performed servicing and maintenance activities on such equipment as but not limited to horizontal bandsaws. The employer did not develop and implement an energy control program, consisting of machine-specific energy control procedures, employee training, and periodic audits exposing employees caught-in and struck-by hazards, on or about July 14, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.219(c)(4)(i):Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: (a) 5278 Brush Valley Road Rebersburg, Pennsylvania, LLC, "RUN-A-GADE" pallet dismantling - On or about July 14, 2022. employees used the green horizontal bandsaw system to disassemble pallets, the rear side had two unguarded shaft ends. (b) 5278 Brush Valley Road Rebersburg, Pennsylvania, On or about July 14, 2022, employees used the gray double head notcher to cut the stringers of the pallet and were exposed to an unguarded shaft. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
