29 CFR  1910.147(c)(7)(i)(C): All other employees whose work operations are or may be in an area where energy control procedures may be utilized, were not instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out:  On or about July 15, 2022, and at times prior thereto, employees were exposed to the hazard of released residual energy from machines , while working in the area of locked out machines, without adequate training.

29 CFR  1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l) through a certification that included the name of the operator, the date of the training, that date of evaluation, and the identity of the person(s) performing the training or evaluation:   On or about July 15, 2022, and at times prior thereto, employees were exposed to struck-by and ejection hazards, while operating the forklifts without being certified.

29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:  a) On or about July 15, 2022, and at times prior thereto, employees were exposed to amputation hazards, while working on the the Chicago press brake that was not guarded at the point of operation. b)  On or about July 15, 2022, and at times prior thereto, employees were exposed to amputation hazards, while working on the Solberga drill press that not guarded at the point of operation. c)  On or about July 15, 2022, and at times prior thereto, employees were exposed to amputation hazards, while working on the Speedmeister drill press that was not guarded at the point of operation.
