29 CFR 1910.119(d)(3)(i)(E): The employer did not have a compilation of written process safety information pertaining to the ventilation system design: a) Machine room: On or about August 17, 2022, the employer did not have a compilation of written process safety information pertaining to the cooling capacity for the two glycol air evaporator units therefore proper ventilation temperature control calculations for the machine room could not be performed. Failure to include proper ventilation information of the machine room can lead to toxic vapors accumulating and employee exposure to ammonia inhalation hazards resulting in serious illness and death. Abatement Certification Required.

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: a) Production area: On or about August 17, 2022, the employer did not document that the equipment complied with the employers chosen RAGAGEP IIAR 2(2014 Addendum A) section 5.14.6 when the ammonia piping to evaporators was not labeled to identify the physical state of the refrigerant and direction of flow. Failure to label ammonia refrigerant piping can expose employees to hazardous ammonia vapors or liquid during maintenance or emergency procedures resulting in serious injuries and possible death Abatement Observed During Inspection.

29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment: Outside Pit Area: On or about July 15, 2022, the employer did not establish written procedures to maintain the on-going integrity of process equipment in that procedures had not been developed or documented to maintain the high pressure receiver (HPR) isolation valve HPL V2-E HV1G. Failure to maintain this valve resulted in an ammonia release during pump out operations exposing employees to an ammonia inhalation hazard. Abatement Certification Required.
