29 CFR  1910.95(d)(1):When information indicated that any employee's exposure equaled or exceeded the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  a) 2100 South Fwy Rd, Pueblo, CO 81004: On or about July 18, 2022, employees were exposed to ear damaging noise levels. The employer failed to conduct noise level measurements and conduct noise sampling on employees. Noise inside the Seamless mill exceeded 90 dba and noise inside the Heat Treat pulpit of the Seamless mill exceeded 85 dba. The employees do not wear earplugs inside the Heat Treat pulpit. The employer failed to conduct measurements including personal sampling and/or sound level meter sampling at the facility to evaluate higher noise level areas and ensure all break rooms and pulpits are safe to not wear earplugs.  b) 2100 South Fwy Rd, Pueblo, CO 81004: On or about July 15, 2022, employees were exposed to ear damaging noise levels. The employer failed to conduct noise level measurements and conduct noise sampling on employees. Noise inside Steelmaking exceeded 90 dba. Employees do not wear earplugs in the Furnace Floor pulpit or the Furnace Floor break room. The employer failed to conduct measurements including personal sampling and/or sound level meter sampling at the facility to evaluate higher noise level areas and ensure all break rooms and pulpits are safe to not wear earplugs.  c) 2100 South Fwy Rd, Pueblo, CO 81004: On or about July 22, 2022, employees were exposed to ear damaging noise levels. The employer failed to conduct noise level measurements and conduct noise sampling on employees. Noise inside the Rail mill exceeded 90 dba. The employer failed to conduct measurements including personal sampling and/or sound level meter sampling at the facility to evaluate higher noise level areas and ensure all break rooms and pulpits are safe to not wear earplugs.  d) 2100 South Fwy Rd, Pueblo, CO 81004: On or about July 18, 2022, employees were exposed to ear damaging noise levels. The employer failed to conduct noise level measurements and conduct noise sampling on employees. Noise inside the Rod and Bar mill exceeded 90 dba. Employees wear earplugs, but sampling did not occur to evaluate higher noise level areas and ensure all break rooms and pulpits are safe to not wear earplugs. The employer failed to conduct measurements including personal sampling and/or sound level meter sampling at the facility.

29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) 2100 South Freeway Pueblo, CO 81004 (EVRAZ): On or about July 17, 2022, employees were exposed to corrosive chemicals which can cause eye damage. The employer failed to provide adequate eyewash stations throughout the facility where corrosive chemicals are regularly handled and utilized. Per the Safety Data Sheet, "Graymont Dolomitic Quicklime requires one to immediately flush eyes with plenty of water. Continue to rinse for at least 20 minutes." Graymont Dolomitic Quicklime is classified as a Category 1 Corrosive (in reference to Table A.3.2 from 29 CFR 1910.1200 Appendix A). There were no areas within close proximity for employees to flush eyes in case of contact.
