29 CFR  1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  (a) 2100 South Fwy Rd, Pueblo, CO 81004 (EVRAZ): On or about July 15, 2022: employees were exposed to struck by hazards in the welding area of Steelmaking. The employer failed to ensure gas cylinders were securely and safely stored. There was a freestanding gas cylinder (oxygen) that was not securely fastened to avoid possibly knocking it over and turning the cylinder into a dangerous propelled object.

The employer failed to ensure there was a chuck guard on the 24 inch Bradford Geared Head lathe.  (b) 2100 South Fwy Rd, Pueblo, CO 81004 (EVRAZ): On or about July 20, 2022: employees were exposed to struck by hazards in the fabrication/boiler shop. The employer failed to ensure there was a chuck guard on the lathe in the fab shop (largest lathe in the building).  (c) 2100 South Fwy Rd, Pueblo, CO 81004 (EVRAZ): On or about July 22, 2022: employees were exposed to struck by hazards in Rail Mill (roll line south of number 1 stand). The employer failed to ensure there was covers over two rotating motors beneath the roll line.

The employer failed to ensure that the gap between the Metabo DSD 250 (bench grinder) grinding wheel and the work rest was no more than one-eighth inch.
