The employer did not ensure a safe means of access to scaffold working platforms. Employees utilized tubular welded scaffolding approximately 30 ft. above the ground without an access ladder to each working level, while installing metal framing on a residential building under construction. Employees were observed using the scaffold frames and cross braces as a means of access. Violation observed 07/19/2022

The employer did not ensure scaffolds were inspected before use. Employees worked from tubular welded scaffolds, approximately 30 feet above the ground, without the scaffold and scaffold components being inspected for visible defects by a competent person. Employees installing metal framing on a residential building used the scaffolds which were missing proper guardrails on all sides for approximately one week without daily inspections. Violation observed 07/19/2022
