29 CFR  1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area:    A.)  Parker-LORD Corporation; 601 S. Street, Saegertown, PA - On or about July 19, 2022, and at times prior thereto, an employee was working with Xylene and Cabosil powder while conducting mixing operations in Tank 5, Building 8.  As the Xylene and Cabisol were being added to the tank at the same time, a flash fire occured, injuring an employee.  The Hazard Communication training provided to employees did not effectively include the physical hazards, including fires, dusts and static discharge, related to mixing the substances.
